Jackson Therapy Partners is seeking a travel Home Health Physical Therapist for a travel job in Springfield, Illinois.

  • Active State License is Required to Start the Assignment
  • BLS Certification May Be Required from AHA or ARC
  • Experience with HCHB is required

Assignment Details

  • 40 expected hours per week
  • Coverage area includes Morgan, Cass, Schuyler, Scott, Brown, Adams, and Pike counties
  • Key duties include patient visits, oversight of PTA, care coordination, team meetings, and case management
