Description

American Traveler is seeking an experienced Home Health RN with Homecare Homebase (HCHB) charting experience for a 13-week travel assignment covering a multi-county territory in Kentucky.

Details
  • Home health care setting serving adult patients across Marion, Washington, Taylor, LaRue, Hardin, Meade, and Breckenridge Counties in Kentucky
  • Caseload of approximately 30 adult patients with a point target of 6.5 points per day
  • Responsibilities include start of care (SOC), recertifications, and daily visits
  • Schedule is Monday–Friday, 8am–5pm, with a rotating weekend shift (8am–5pm)
  • No on-call requirements
  • Clinical support available from a clinical manager or educator 24/7 for emergencies, and 8am–5pm for general needs
  • EMR: Homecare Homebase (HCHB) — experience required
Requirements
  • Active KY RN license or compact (NLC) license currently in hand — pending licenses not accepted
  • Current BLS certification required — pending certifications not accepted
  • Minimum 1 year of Med/Surg experience in a hospital setting required
  • Home health experience required — no exceptions
  • Homecare Homebase (HCHB) charting experience required — no exceptions
  • Two professional references required: one supervisor from within the last 12 months, and one peer or supervisor from within the last 3 years — must include dates of employment and eligibility for rehire
  • Copy of driver's license required for consideration
Additional Information
  • Orientation consists of approximately 2 weeks working alongside a workcoach on initial visits
  • Phone and tablet will be provided for use in the field
  • Holiday rotation required
  • Scrubs required (no color restriction)
