JOB SUMMARY:
The Registered Nurse Part Time provides personalized, high-quality nursing care to patients in their homes. This role involves assessing patients' needs, developing care plans, educating and delivering a variety of medical services to support the patient's recovery needs, health management, and patient comfort on an as needed basis. The Registered Nurse collaborates with patients, families and other healthcare professionals to ensure optimal care and outcomes.
ESSENTIAL JOB DUTIES:
  • Initiates, implements, and supervises patient plan of care.
  • Demonstrate the ability to observe, record and report the needs and concerns of the client.
  • Coordinate the needs of home health clients and maintain constructive relationships with them and with all care providers.
  • Follows the approved Plan of Care.
  • Demonstrates accurate and thorough documentation skills and compliance with documentation guidelines.
  • Maintains productivity standards as defined by the Leadership Team.
  • Monitor client's progress towards stated goals and communicate with upline clinical manager as needed for adjustments to the Plan of Care.
  • Performs medication review and reconciliation
  • Evaluates the appropriateness, cleanliness, and safety of durable medical equipment, supplies and/or appliances.
  • Assists the client in learning appropriate self-care techniques.
  • Communicates with other members of the health care team.
  • Adheres to established policies and procedures.
  • Completes company and state mandated forms and training in a timely manner.
  • Maintain a professional attitude and dress appropriately.
  • Be responsive to members of Northshore Home Health when they contact and keep the company aware of the current schedule and any changes.
  • Inform the office of all changes with client, personal health and scheduling.

QUALIFICATIONS:
  • Graduate of an approved RN Program.
  • State Licensed and currently registered as a Registered Professional Nurse.

WORK ENVIROMENT:
• Ability to travel to patients' homes in coverage area.
• Must be able to lift, move or assist patients as needed.

CERTIFICATIONS/LICENSES:
• Valid Nursing Degree.
• Valid Nursing License.
• Valid CPR Certification.
