$15,000 Sign-On Bonus (Full-Time) | $7,500 (Part-Time)

About the Role

To The Rescue is seeking a Registered Nurse (RN) โ€“ Home Health Case Manager to provide skilled nursing care and care coordination for adult patients in their homes. This role offers meaningful one-on-one patient care, professional autonomy, and a healthier work-life balance compared to hospital or facility-based nursing.

Youโ€™ll manage patient care from admission through discharge, working collaboratively with physicians, therapists, patients, and families to help individuals remain safe, independent, and comfortable at home. We offer a supportive team environment, realistic expectations, and a $15,000 sign-on bonus for full-time RNs.




What Youโ€™ll Do
  • Provide skilled nursing visits in accordance with physician orders and individualized plans of care

  • Complete comprehensive patient assessments and ongoing evaluations

  • Develop, implement, and update care plans in collaboration with the care team

  • Coordinate services with physicians, therapists, patients, and caregivers

  • Educate patients and families to support safety, independence, and health outcomes

  • Document care accurately and timely according to agency and regulatory standards

  • Active Registered Nurse (RN) license in the state of Iowa
