Job description

Health Home RN

The Health Home RN is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as case finding, health teaching and counseling. The nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. The role will require on call coverage and a willingness to be flexible.

Essential Functions:

  • Based on a comprehensive review of the client's overall condition, prepares, continually reviews and evaluates plans of nursing intervention to ensure that such interventions are consistent with the client's comprehensive treatment plan. Ensures that plans of nursing intervention include consideration of the client's cultural and ethnic background, customs, needs, beliefs, and primary language.
  • Develops nursing assessment to reflect holistic needs of each client with consideration of his/her psychological, physiological, social, ethnic, cultural, educational and spiritual strengths and needs.
  • Oversees the direct implementation of nursing care to ensure that it is carried out in accordance with the plan of nursing intervention and overall treatment/service plan and to ascertain if the needs of the client require modification of the plan.
  • Participates at team meetings and provides nursing input on the development and implementation of treatment/service plans.
  • Coordinates nursing care activities with other professional disciplines to ensure that the nursing plan is consistent with and integrated with the total treatment/service plan.
  • Provides information to other clinical staff on nursing standards of practice and procedures.
  • Provides advice and consultation on nursing related matters to supportive housing staff and administrators.
  • Provides health education/teaching for clients and their families.
  • Conducts counseling sessions, with direction as needed by other qualified professionals, according to principles of communication, human behavior and psychiatric nursing.
  • Manages medical supplies and equipment.
  • Accesses medications and observe and records effects and side effects.

Non-Essential Functions:

  • Transports and/or accompanies clients as needed.
  • Ensure the safety of the clients at all times.
  • Keep Agency vehicle clean and documents use of Agency vehicle as required.
  • Participates in risk management and quality assurance concerning health related issues.
  • Participates in team and administrative meetings.
  • Provides emergency care and crisis intervention as needed.
  • Participate, as needed, in Agency response to serious incidents.
  • Performs other related duties as assigned.

Specifications for Education/Licenses/Certifications:

  • Associates Degree in Nursing or equivalent.
  • Bachelor's Degree Preferred.
  • License and current registration to practice as a Registered Nurse in New York State.
  • Valid NYS Driver License required. 3 years of current driving experience with no more than one moving violation in the previous 24 months (2 years), and no driving while intoxicated convictions required.

Specifications for Experience and Training:

  • One year post RN licensure clinical nursing experience working with individuals with mental disabilities preferred.
  • Bilingual Spanish speaking preferred.
  • Clinical Experience preferred.
  • Excellent oral, writing and communication skills.
  • Ability to teach skills to individuals and groups.
  • Free from present litigation that could result in loss of licensure.
  • Capacity to learn computer skills.
  • Ability to perform skillfully, communicates clearly, discuss assessments with clients.
  • Current knowledge of psychopharmacology and side effects.

Specifications for Physical Requirements:

  • Ability to work flexible hours and days – including weekends/evenings/holidays according to needs of a 24/7 program.
  • Regularly required to talk, hear, walk, stand & sit.
  • Able to lift up to 10 pounds.
  • Able to climb stairs and make home visits.
  • Able to stretch and bend to retrieve files.
  • Able to operate a computer keyboard, mouse, & office equipment.
