Responsibilities

DESCRIPTION:
Under the direction of the clinical manager, the Home Health Registered Nurse performs primary nursing care functions in conformance with professional standards of home health nursing practice.

Providing individualized care to patients in their home under the plan of care approved by the primary physician. The home health nurse coordinates all services provided for each assigned patient and family.

The Home Health Registered Nurse provides leadership and advocacy in the coordination of patient care with multiple clinicians, modalities, and community resources. They manage clinical resources and transition planning for patients within the assigned caseload from pre-admission through post-discharge, actively working to provide excellent medical care.
