We are looking for a clinically experienced nurse who has nursing documentation/charting skills and is a fast learner with great communication skills.

This opportunity is open for a highly motivated and positive-minded individual.

Your Role in our Company Will Be:

The RN travels to a patient's home to administer their services and helps patients maintain their independence. Their duties include: Administering at-home IVs, changing dressings, cleaning wounds, in-depth assessments, overseeing patient treatment plans, As well as Updating Doctors & The Agency about their patient's health. Documentation and communication with the Agency is done through Synergy Software.

AVAILABILITY:

Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two-three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer)

Work Area: City close to you, with a group of patients who need care for that day

RESPONSIBILITIES

  • Providing nurse visits to home health patients. (home visits)
  • Performing an initial, comprehensive assessment. Documenting everything & Uploading to Agency Portal
  • Obtaining and documenting physician orders.
  • In consultation with the assigned Clinical Team Manager, you will need to initiate and regularly re-evaluate and revise the plan of care
  • Coordinating care with the assigned Clinical Team Manager, physician and other members of the home care team, informing them of significant changes in the patient’s condition and needs.
  • Performing re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager. Completing and submitting all related documentation
  • Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA).
  • Performing Home Health Aide supervisory visits per regulatory requirements.
  • Demonstrate knowledge of regulatory requirements.

POSITION QUALIFICATIONS

  • Visiting RN for Home Health: 1 year (Required)
  • Nursing: 1 year (Required)
  • License/Certification:
  • Current CA RN License (Required)
  • BLS from the American Heart Association and/or the American Red Cross (Required)
  • NSO Nursing License Insurance (Required)
  • IV/Phlebotomy Certified (Required)
