Registered Nurse (RN)

We are seeking a dedicated Registered Nurse (RN) for a Home Health position in the West Portland, OR area. This role requires skilled experience in home health care and is ideal for compassionate individuals who can provide high-quality patient care in a home setting.

Key Responsibilities

  • Provide one-on-one patient care in a home health environment.
  • Manage up to six patient visits per day while maintaining a productivity standard of 30 points.
  • Conduct assessments, develop care plans, and implement nursing interventions.
  • Utilize electronic health record systems, including EPIC Remote Client and Oasis.
  • Transport patients as needed, ensuring minimal travel time by working within grouped client allocations.

Requirements

  • A valid Oregon nursing license is required.
  • Minimum of one year of skilled home health experience is required.
  • Basic Cardiac Life Support (BCLS) certification is mandatory.
  • Primary series COVID vaccination and one booster are required.
  • Experience with Oasis documentation is essential.

Work Schedule

  • Shifts: 8-hour day shifts, Monday through Friday.
  • No weekend commitments.
