Job Title: Registered Nurse โ€“ Clinical Manager (Home Health)

Job Summary

We are seeking an experienced and detail-oriented Registered Nurse (RN) to join our team as a Clinical Manager for Home Health. The ideal candidate will have a strong clinical background in home health, excellent leadership skills, and hands-on experience with EMR systems, preferably Kinnser (WellSky). This role is responsible for overseeing clinical operations, ensuring compliance, and maintaining high-quality patient care.

Key Responsibilities

Oversee day-to-day clinical operations of home health services

Supervise and support field clinicians including RNs, LVNs, PTs, OTs, and aides

Review, approve, and ensure accuracy of OASIS documentation and care plans

Ensure compliance with state, federal, and accreditation standards

Coordinate patient care services and ensure timely admissions, recertifications, and discharges

Monitor clinical outcomes and implement quality improvement initiatives

Provide clinical guidance and training to staff

Collaborate with physicians, referral sources, and interdisciplinary teams

Ensure proper utilization of EMR systems for documentation and workflow efficiency

Participate in audits and ensure readiness for surveys

Qualifications

Active Registered Nurse (RN) license in [State]

Minimum 3+ years of home health experience (mandatory)
