As an RN Case Manager, you'll assess patient needs, provide education and support, coordinate care, and help patients thrive at home.

In this role, you will build meaningful relationships with patients and caregivers, use your clinical judgment to guide care, and help people remain safe and supported in their homes.
As an RN Case Manager, you will serve as a trusted clinical partner to patients, families, caregivers, physicians, and members of the broader care team. You will assess patient needs, develop individualized plans of care, monitor changes in condition, provide education and support, and coordinate the services and resources needed to help each patient succeed at home.

What You’ll Do
Conduct in-home clinical assessments and help patients and caregivers successfully begin services.
Develop, implement, and update individualized plans of care based on each patient's needs, goals, and clinical condition.
Build ongoing relationships with patients and caregivers through scheduled home visits and regular clinical follow-up.
Use your clinical judgment to identify changes in condition, safety concerns, and barriers to successful care.
Coordinate care with physicians, caregivers, internal clinical teams, and community resources.
Educate and coach patients and caregivers on care plans, best practices, health and safety needs, and available resources.
Review caregiver documentation and respond appropriately to clinical changes or concerns.
Advocate for patients and help ensure they receive safe, effective, person-centered care.
Maintain accurate clinical documentation and ensure care is delivered in accordance with applicable regulatory requirements and company standards.
Collaborate with a supportive interdisciplinary team to improve patient outcomes and the overall care experience.
What We’re Looking For
Graduate of an accredited nursing program.
Active and unrestricted Registered Nurse license in the State of Connecticut
Strong clinical assessment, communication, and documentation skills.
A compassionate, patient-centered approach to care.
The ability to communicate effectively with patients, families, caregivers, physicians, and other members of the care team.
Comfort working independently while remaining closely connected to a collaborative clinical team.
Ability to travel by car within an assigned geographic area.
Valid driver's license and active auto insurance.
Comfort using common clinical, case management, and productivity software.
New graduates are encouraged to apply.
Experience in home care, community health, case management, or care coordination is helpful, but not required.
