As of Aug 11, 2026, the average weekly pay for from home rn in Champaign, IL is $1,958.94, according to ZipRecruiter salary data. Most workers in this role earn between $1,530.77 and $2,292.31 per week, depending on experience, location, and employer.

What are some common challenges faced by from home RNs, and how can they overcome them?

Work-from-home RNs often face challenges such as maintaining clear communication with patients and healthcare teams, managing time effectively without direct supervision, and ensuring patient privacy in a remote setting. To overcome these, it's important to establish a dedicated, private workspace, use secure telehealth platforms, and stay organized with digital tools. Regular check-ins with team members and ongoing training in telehealth best practices can also help remote RNs deliver high-quality care and feel connected to their colleagues.

What is a from home RN?

A work from home RN (Registered Nurse) is a licensed nurse who performs their job duties remotely, often through telehealth platforms. These nurses provide care, support, and education to patients over the phone or via video calls instead of in-person visits. Their responsibilities may include triage, case management, patient education, and coordinating care with other healthcare professionals. This role allows nurses to use their clinical expertise without being physically present in a healthcare facility.

Description

Coverage Territory:  Champaign, Piatt and surrounding areas

Schedule:  8 AM-8 PM, alternating 4-day and 3-day weeks including weekends in rotation

Make the Moments That Matter Most

At Transitions Hospice, we don't just manage care - we protect dignity, comfort, and the moments that matter most. We've reimagined hospice to be something different: compassionate, unhurried, and built around the whole person. That means 24/7 care delivered where patients feel safest - at home - with physical, emotional, and spiritual support that lets them focus on living.

If you're an RN who wants your work to mean something beyond a chart, this is your role.

What You'll Do

As an RN Case Manager, you'll be the clinical anchor for your patients and their families - coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:

  • Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
  • Building and adjusting personalized care plans in collaboration with physicians and care teams
  • Administering medications and treatments per physician orders
  • Keeping clinical documentation accurate, timely, and meaningful
  • Serving as the communication bridge between families, physicians, and community health partners
  • Educating patients and caregivers so they feel confident and supported
  • Participating in on-call rotation per team policy

Benefits We Offer

  • Competitive pay
  • Medical / Dental / Vision Insurance plans
  • Long & Short-Term Disability
  • 401(k) with company match
  • Auto Allowance & Gas Card
  • Company provided equipment
  • Paid Time Off

Why Choose Transitions Hospice?

Hospice nurses at Transitions don't just manage cases - they change lives. We give you the tools, support, and team to do your best work, without sacrificing the human side of care. Our nurses are the difference, and we treat them that way.

Ready to join us? Apply today.

  • Compassion for patients and families navigating end-of-life care
  • Strong clinical judgment and the ability to work independently in the field
  • Excellent communication skills across care teams and families
