Job description

We are seeking an experienced Home Health Registered Nurse to serve as our Clinical RN Manager, providing clinical leadership and oversight throughout Mahoning, Trumbull, and Columbiana counties.

The ideal candidate will have strong home health experience, including OASIS assessments, plans of care, clinical documentation, Medicare compliance, care coordination, and supervision of field staff.

This hands-on leader will oversee patient admissions, recertifications, transfers, and discharges; review clinical records and physician orders; monitor patient outcomes and changes in condition; manage caseloads and visit completion; and supervise, coach, and evaluate RNs, LPNs, and Home Health Aides.

Candidates must hold an active Ohio RN license and demonstrate excellent clinical judgment, organization, accountability, and communication. If you are an accomplished home health RN ready to lead a clinical team while ensuring safe, compliant, patient-centered care, we encourage you to apply.
