Job description

Description
JOB SUMMARY:
The RN is responsible for initiating and execution of care plans for patients upon receiving physician orders for home health care and regularly reassesses and coordinates the patient's medical needs.
JOB DUTIES AND RESPONSIBILITIES
1. Performing comprehensive assessments of patients at admission, transfer, discharge, and recertification by providing evaluations and accurately assessing and answering OASIS questions. The documentation for these activities must be completed and in "submitted with signature" status within 2 days of performing the visit. (EF)
2. Perform skilled nursing visits with aide supervision as needed and "hi-tech" skilled nursing visits which may include visiting patients with a wound vac, breathing or eating port, etc. The documentation for this activity must be completed and in "submitted with signature" status within 24 hours of performing the visit. (EF)
3. Acts as Alternate Agency Disaster Coordinator in the absence of the Director or Alternate. (EF)
4. Delegates duties to aides and LVNS. Supervises aides and LVNs. (EF)
5. Provides patient care under the treatment plan ordered by the physician and documents according to regulations. (EF)
6. Participate in the On-Call rotation. (EF)
7. Provide infection and occurrence reports when needed. (EF)
8. Assists with clinical record reviews. (EF)
9. Participates in the PI program. (EF)
10. Assist the RN Case Managers when needed. (EF)
11. Complies with all Home Health State and Federal Regulations as applicable (EF)
Hospital:
1. Adheres to organization-wide and other applicable policies and procedures.
2. Day-to-day performance complies with the hospital's Service Excellence Behavioral Expectations.
3. Performs within the prescribed limits of the hospital's Ethic, Compliance and Confidentiality Program guidelines.
4. Performs within professional Scope of Practice at all times.

Minimum Requirements
EDUCATIONAL REQUIREMENTS
Required: A.D.N., B.S.N., Diploma Degree in Nursing
EXPERIENCE
Required: 6 months in a hospital setting
Preferred: 1 year in a hospital setting and previous home health experience
Special Skills and Abilities:
Required: Supervisory skills, computer skills, communication skills, ability to perform well in stressful situations.
Preferred: Wound care skills, central line skills
LICENSURE / CERTIFICATION REQUIREMENTS
Required: Current License Registered Nurse by the Texas Board of Nursing, Driver's License, and CPR certification required.
