Position Details

  • Contingent, per-visit pay structure
  • Orientation and additional training compensated at a separate hourly rate
  • Competitive wages

Responsibilities

  • Conduct initial and ongoing comprehensive patient assessments, including OASIS evaluations
  • Complete assessments within 24–48 hours of receiving an initial referral
  • Develop, implement, and revise patient care plans in collaboration with physicians
  • Provide skilled nursing treatments as outlined in the plan of care
  • Educate patients and caregivers using clear, understandable instruction
  • Document evaluations, treatments, and patient responses accurately and in a timely manner
  • Participate in discharge planning with staff, physicians, and patients
  • Attend case conferences and required meetings
  • Supervise LPNs in accordance with agency policy
  • Ensure compliance with all Federal, State, Medicare, Medicaid, and Accreditation requirements

Minimum Qualifications

  • Active Registered Nurse license in good standing in the State of Michigan
  • Minimum of two (2) years of nursing experience (home health experience preferred)
  • Knowledge of Medicare/CMS guidelines
  • Proven experience completing OASIS assessments
  • Experience with EMR systems (Kinnser preferred)
  • Reliable transportation, valid Michigan driver’s license, and proof of auto insurance
  • COVID-19 vaccination required
  • BLS certification through AHA or Red Cross
