Job description

Registered Nurse (RN) - Archbold Inpatient Hospice

Position Description:

Provide Hospice-specific nursing care to patients in homes, nursing homes, assisted living facilities or Hospice Inpatient Unit

Provide initial and ongoing counseling/education regarding illness, disease process, and/or bereavement support

Perform comprehensive Nursing Assessment on newly admitted patients

Evaluate data collection, formulate a plan of care and establish specific goals for the patient and family/support system

Work closely with physicians communicating pertinent observations

Regularly reassess the patient and update the care plan accordingly

Coordinate supplies and durable medical equipment, assuring delivery in a timely and cost-efficient manner

For inpatient and outpatient hospice care;  coordinate inpatient, respite and acute admissions

For outpatient hospice care; coordinate outpatient services and treatments

Serves as patient/family advocate

Maintain patient/family dignity

Experience/Qualifications:

Current GA RN license

At least two years of experience, preferably in a hospice setting

For outpatient Hospice services, it may include extensive travel in the service area (must have clean driving record)
