Responsibilities

  • Perform initial home health evaluations and ongoing reassessments as needed.
  • Develop and implement individualized plans of care.
  • Complete OASIS documentation accurately and in a timely manner.
  • Supervise Home Health Aides (HHAs) and provide clinical guidance as appropriate.
  • Ensure compliance with Medicare regulations and agency policies.
  • Communicate changes in patient condition to physicians and other healthcare providers.
  • Coordinate care with interdisciplinary team members to support optimal patient outcomes.
  • Complete all required documentation and paperwork efficiently and on schedule.
  • Educate patients and caregivers regarding disease processes, medications, treatments, and care plans.
  • Deliver high-quality nursing care in accordance with professional standards and agency guidelines.
  • Provide skilled nursing services, including wound care, IV therapy, line maintenance, and ongoing assessments.
  • Collaborate effectively with healthcare professionals, patients, and family members to ensure continuity of care.
Qualifications
  • Current and unrestricted Illinois Registered Nurse (RN) license.
  • Home health experience preferred.
  • Proficiency with OASIS documentation.
