Alpha Home Health & Hospice of King County is growing and seeking compassionate, skilled Registered Nurses (RNs) to join our Home Health team. This is a great position for entry to the industry or for new/newer RNs.

What You'll Do

As a Home Health RN Case Manager, you will provide skilled nursing care to patients in their homes while coordinating and managing their overall plan of care.

Key Responsibilities
  • Perform comprehensive patient assessments and ongoing nursing evaluations
  • Develop, implement, and update individualized care plans
  • Provide skilled nursing services, including medication management, injections, wound care, and treatments
  • Monitor patient progress and communicate changes in condition to physicians and care team members
  • Educate patients and caregivers on diagnoses, medications, treatment plans, and disease management
  • Coordinate care with physicians, therapists, social workers, and other healthcare professionals
  • Complete accurate and timely EMR documentation
  • Ensure compliance with Medicare, Medicaid, state, and agency regulations
  • Participate in interdisciplinary team meetings and case conferences
  • Promote patient safety, independence, and quality of life
Qualifications
  • Active Washington State RN License
  • Current BLS Certification
  • Valid driver's license and insured vehicle
  • Home Health or Hospice experience preferred, but not required
  • Strong clinical, communication, and organizational skills
