Field Medical Assistant - Patient Transitions & Connected Care
Remote Care Partners
HealthyConnect | East Tampa and Polk County, Florida
Full-time | Weekdays | $21-$24 per hour plus mileage and benefits
HealthyConnect is hiring personable, dependable Medical Assistants to visit patients in their homes after discharge from a hospital or skilled nursing facility.
This is not a traditional office-based Medical Assistant position. You will help recently discharged patients connect with their HealthyConnect clinical team and set up simple technology used to support their recovery at home.
What you will do
Complete approximately four to six scheduled home visits each weekday.
Travel within a relatively compact assigned territory in East Tampa and Polk County.
Deliver, install and test lightweight wireless health-monitoring equipment.
Teach patients and caregivers how to use the equipment.
Confirm that readings are successfully reaching HealthyConnect's centralized patient monitoring team.
Help patients connect with nurses, nurse practitioners and other clinical personnel for telehealth visits.
Obtain and document vital signs and other information within the Medical Assistant's authorized scope.
Identify medication-list discrepancies, changes in condition or other patient concerns and promptly escalate them to the appropriate clinician.
Document each visit accurately using a company-provided mobile device.
Maintain accountability for equipment, supplies and replacement inventory.
Provide a calm, supportive experience for patients and families during the transition home.
What we are looking for
Medical Assistant training or relevant patient-care experience.
CMA or RMA certification required.
Exceptional interpersonal skills, patience and comfort working with older adults.
Confidence using smartphones, tablets, Bluetooth and wireless devices.
Ability to work independently while remaining connected to a remote clinical team.
Strong organization, punctuality and documentation skills.
Reliable personal transportation, a valid Florida driver's license and current automobile insurance.
Ability to carry and set up a lightweight equipment kit, generally weighing approximately five pounds.
Willingness to travel within a relatively compact assigned service territory.
Ability to pass required background screening and driving-record review.
Bilingual English/Spanish capability is helpful but not required.
Schedule
Full-time, weekday position.
No routine nights or weekends.
Approximately four to six patient visits per day.
Patient visits are grouped geographically whenever practical to limit unnecessary driving.
Paid training is provided before independent patient visits begin.
Travel time between scheduled patient visits is paid.
