Home Health - Registered Nurse (RN) / Licensed Practical Nurse (LPN)

Coverage Area: Chicago, IL

Full-Time | Part-Time | PRN — You choose your pace!

$65,000 – $100,000+ per year. You set the ceiling!

Where You'll Work

Field-based role — you'll travel to patients in their homes within your assigned area.

We serve patients across: McHenry, Kane, Kendall, Cook, DuPage, Will, Lake, Winnebago, and Boone Counties


What You'll Do
Registered Nurse - Overview:

  • OASIS Start of Care, Recertification and Resumption and case management
  • Assessing or re-evaluating the patient's condition and needs to determine homebound status and plan of care
  • Provide patient care according to patient's care plan and needs
  • Teach the patient and/or family members about care and support skills.
  • Medication management and education for patient and/or family.
  • Communicate regularly with Clinical Care Coordinators, Interdisciplinary team, and overseeing physician.
  • Accurately and thoroughly document patient care within company policy.
  • Supervision of License Practical Nurse and CNAs.
  • Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life.


Qualifications:

  • Active Illinois RN in good standing
  • Valid driver’s license, reliable vehicle, and auto insurance
  • Current CPR Certification
  • Prior experience in home health, hospice, rehab, or hospital setting (required)
  • Familiarity with Electronic Medical Records (EMR) software
  • Comfortable with technology - a company tablet will be provided
  • Strong communication and interpersonal skills
  • Time management to balance patient care, travel, and documentation
  • Commitment to great customer service
