Key Responsibilities
  • Conduct comprehensive assessments of patients to identify their mental health needs and tailor treatment plans accordingly.
  • Facilitate communication between supervisors, direct care staff, families, and other stakeholders to monitor and report on an individual's progress regularly.
  • Develop and maintain a tentative appointment schedule, including home visits and supportive observations, for ongoing patient engagement.
  • Coordinate in-home visits to gather firsthand information through observations and data collection, ensuring that patient assessments are thorough and accurate.
  • Evaluate and ensure the effective implementation of each patientโ€™s program plan, making adjustments as necessary for optimal outcomes.
  • Participate in interdisciplinary team meetings to discuss patient progress and adjust treatment strategies collaboratively.
  • Document monthly and quarterly progress notes to track patient development and inform relevant parties.
  • Organize training sessions to ensure that all staff members are well-versed in the program implementation and documentation protocols.
  • Provide ongoing support to clients through follow-up interviews and home visits, ensuring a complete understanding of their evolving needs.
  • Maintain detailed clinical records and reports to ensure that patient information is readily available for clinical decision-making.
  • Act as a liaison between the public, mental health community, and the organization, championing effective practices for mental health development.
  • Contribute to the creation and enhancement of training materials designed to improve programs for clients.
Patient Population / Client Focus

Our LPCs provide care to a wide range of clientele, including both adults and youth, experiencing various mental health challenges. We focus on creating supportive environments where individuals can explore their mental health concerns, set personal goals, and achieve meaningful change. Qualifications
  • A Masterโ€™s or Doctoral degree in Counseling, Psychology, or a related field is required.
  • Possession of an active and unrestricted license as a Professional Counselor (LPC) in the state of Georgia is essential.
  • A minimum of two yearsโ€™ experience in a counseling role, preferably within a clinical or community health setting.
  • A solid understanding of legal, ethical practices, and guidelines governing professional counseling.
  • Exceptional interpersonal skills with a proven ability to establish rapport and trust with clients from diverse backgrounds.
  • Demonstrated ability to work collaboratively within a multi-disciplinary healthcare team.
