Work Arrangement: Field-based hybrid position with potential work-from-home eligibility following successful completion of the initial three-month training period

Summary/Objective

The Case Manager – Registered Nurse works collaboratively with a Social Worker Case Manager to provide case management services to individuals participating in home- and community-based services programs. Responsibilities include pre-admission screening, assessment, service planning, care coordination, monitoring, and arranging appropriate services in a timely and cost-effective manner.

This position provides consultation, case management, and non-skilled services. It does not perform medical procedures or provide skilled nursing care. The employee maintains regular contact with program participants, family members, service providers, healthcare professionals, community organizations, and the public.

Essential Functions

  • Works with a Social Worker Case Manager as part of an interdisciplinary team to screen referred individuals and determine program eligibility.

  • Conducts required in-person visits in participants’ homes and other community settings.

  • Assesses participants using prescribed assessment tools and professional nursing knowledge.

  • Reviews and documents medical diagnoses, functional limitations, service needs, risks, and other relevant information.

  • Develops individualized plans of care based on assessed needs, program requirements, participant preferences, and available resources.

  • Implements plans of care after eligibility is established and required approvals are received.

  • Monitors participants at least monthly, or more frequently when required, to evaluate services, identify changes in condition or circumstances, and determine whether the plan of care remains appropriate.

  • Coordinates services and communicates with participants, family members, providers, healthcare professionals, and other involved parties.

  • Provides technical assistance to service providers involved in participants’ care.

  • Assists with resource development and service coordination to reduce unnecessary duplication, appropriately target services, and maximize the effective use of available resources and information systems.

  • Maintains complete, accurate, timely, and confidential case records in accordance with program requirements and CMPDD policies.

  • Attends required meetings, training sessions, conferences, and case consultations.

  • Complies with applicable laws, regulations, program standards, confidentiality requirements, and CMPDD policies and procedures.

  • Performs other duties necessary for the successful operation of the case management program.

Supervisory Responsibility

Work Arrangement and Remote-Work Eligibility

This is a field-based hybrid position and is not fully remote. All newly hired employees must successfully complete an initial three-month training period before becoming eligible to work from home.

Following successful completion of the training period, an employee may be approved to work from home in accordance with departmental procedures and CMPDD’s remote-work policies. Remote-work eligibility is contingent upon satisfactory job performance, attendance, productivity, timely and accurate documentation, successful caseload management, compliance with confidentiality and information-security requirements, and the employee’s ability to work independently.

Employees must remain available during scheduled work hours and must continue to complete all required home visits, field assignments, meetings, training, and in-office responsibilities. Approval to work from home is not guaranteed and may be modified, suspended, or revoked based on performance, operational needs, program requirements, or policy compliance.

Employees working from home must maintain an appropriate workspace and reliable internet service and must protect confidential participant and organizational information in accordance with HIPAA, program requirements, and CMPDD policies.

Work Environment

The employee works in office, remote, community, and residential environments based on assigned duties and caseload needs. While performing field responsibilities, the employee regularly travels by automobile and may be exposed to changing weather conditions, varying residential conditions, animals, tobacco smoke, and other environmental factors encountered during home visits.
