Private Duty Registered Nurse Case Manager

What You'll Do

As a Private Duty RN Case Manager, you will oversee and coordinate care for patients receiving in-home private duty services.

Responsibilities include:

Assess patient health needs and determine appropriateness for home care admission
Complete admission assessments and develop individualized plans of care
Communicate with physicians, caregivers, and referral sources to ensure smooth care transitions
Coordinate services including equipment, supplies, pharmacy, and additional medical support
Supervise Private Duty RNs, LPNs, and Home Health Aides to ensure quality and continuity of care
Review clinical documentation for accuracy and compliance with the plan of care
Evaluate patient progress and adjust care plans when necessary
Provide education and resources to patients and caregivers

What You'll Bring

Current Registered Nurse (RN) license in Indiana
Graduate of an accredited RN program
2+ years clinical nursing experience preferred
Strong communication and patient care coordination skills
Ability to work collaboratively within an interdisciplinary team
Ability to lift, push, pull, or support up to 50 pounds as needed

Preferred:

Home Health or Private Duty nursing experience
Case management experience
