Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

  • Travel Pay Package: Up to $2,340.00 weekly.

    *(If your home location is more than 50 miles); $1680.00 weekly pay will be paid in the form of non-taxable stipend.

Location: Brooklyn Park, MN, 55428; Coverage area: Local within 20-30 miles radius.

Shift: 8:00 Hours/Day, 09:00 AM to 05:00 PM
We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will assess patient needs, develop and implement individualized Plans of Care, coordinate services with the interdisciplinary team, educate patients and families, and ensure high-quality, compliant patient care.

The ideal candidate will have strong Home Health experience, OASIS expertise, and Homecare Homebase (HCHB) experience, along with the ability to work independently and travel locally.

Key Responsibilities:

  • Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
  • Develop, implement, document, and evaluate individualized Plans of Care (POC).
  • Collect and incorporate physical, psychological, social, spiritual, and cultural information into the patient's care plan.
  • Maintain accurate, timely, and complete clinical documentation and patient records.
  • Provide skilled nursing care while following appropriate infection-control and OSHA standards.
  • Educate patients and families regarding medications, diet, treatment plans, home safety, disease management, and self-care.
  • Supervise and provide clinical direction to HHA/STNA/LVN staff to ensure continuity and quality of care.
  • Assess and reassess pain and implement appropriate pain-management interventions.
  • Coordinate referrals to appropriate agencies and healthcare providers as needed.
  • Monitor assigned cases for compliance with third-party payer requirements.
  • Participate in assigned on-call responsibilities and provide support to patients and families as required.
  • Serve as a liaison between patients/families, physicians, administration, and interdisciplinary team members.
  • Participate in interdisciplinary team meetings and other required meetings.
  • Perform other related duties as assigned.

Required Skills & Competencies:

  • Strong clinical assessment and patient-care skills.
  • Ability to respond appropriately to patient and family needs in varied home-care settings.
  • Excellent verbal and written communication skills.
  • Knowledge of medications and safe medication administration.
  • Knowledge and competency in venipuncture and IV care.
  • Competency in wound assessment, measurement, treatment of surgical wounds and pressure injuries.
  • Ability to administer inhalation therapy and perform phlebotomy for outpatient laboratory testing.
  • Ability to develop and initiate bowel and bladder training programs when appropriate.
  • Experience with indwelling catheters, drains, and other home-care equipment.
  • Ability to safely utilize durable medical equipment commonly used in the home.
  • Strong organizational and time-management skills.
  • Ability to prioritize tasks and work effectively in stressful situations.
  • Ability to work independently as well as collaboratively with an interdisciplinary team.
  • Commitment to professional growth, competency, and continuing education.
  • Ability to maintain patient confidentiality.
  • Flexibility to travel locally and work a flexible schedule.

Required Qualifications:

  • Current state RN license – Required
  • 1–2 years of Registered Nurse experience in a clinical care setting – Required
  • 2+ years of Home Health experience – Required
  • Homecare Homebase (HCHB) experience – Required
  • OASIS experience – Required
  • Current CPR certification – Required
  • Current driver's license – Required
  • Reliable transportation and valid auto insurance – Required
  • Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program – Required
