What You'll Do

  • Complete comprehensive Start of Care (SOC) assessments in patients' homes.
  • Accurately complete OASIS documentation and establish individualized plans of care.
  • Educate patients and caregivers about diagnoses, medications, and home health services.
  • Coordinate care with physicians, therapists, case managers, and other members of the care team.
  • Ensure smooth transitions from hospital, rehabilitation, or skilled nursing facilities to home.
  • Maintain timely and accurate documentation.

What We're Looking For

  • Active Minnesota Registered Nurse (RN) license.
  • Strong clinical assessment and critical thinking skills.
  • Home health and OASIS experience preferred.
  • Excellent communication and organizational abilities.
  • Ability to work independently in a field-based role.
  • Reliable transportation.
