Registered Nurse - Home Care
Job Summary
Provides skilled nursing care to patients in their homes, managing chronic and acute conditions while promoting independence and safety. Conducts comprehensive assessments, develops individualized care plans, and coordinates with physicians and other home health disciplines.
Education & Experience
• Associate's or Bachelor's Degree in Nursing (Required)
• Minimum 1-2 years of med-surg, home health, or community health nursing experience (Preferred)
Licenses & Certifications
• Current RN license in state of practice (Required)
• Basic Life Support (BLS) certification (Required)
• Valid driver's license and reliable transportation (Required)
Key Responsibilities
• Conducts comprehensive in-home patient assessments, including OASIS documentation as applicable.
• Develops and updates individualized plans of care in collaboration with physicians.
• Administers medications, wound care, and other skilled nursing interventions in the home setting.
• Educates patients and caregivers on disease management, medications, and safety.
