Job description

At Fedelta Home Care our mission is to make in-home care possible by providing flexible services that help seniors thrive in their familiar surroundings, surrounded by the people and routines they love. We focus on delivering personalized and professional assistance, from help with daily tasks to skilled nursing care, with the goal of ensuring clients receive outstanding care with comfort, dignity, and peace of mind.

As a Per Diem Registered Nurse (RN) you'll be responsible for collaborating on patient care plans and assisting with training of new Caregivers through evaluation and return demonstrations.

Essential Duties:

Under the supervision of the Home Caregiver Regional Manager, this position is responsible for, but not
limited to:
- Conducting initial in-home prospective patient evaluations and/or assessments.
- Establishing patient's individualized service plan and client record baseline.
- Collaborating with agency staff and contract services to implement the patient's service plan.
- Supervising the implementation of the service plan and the quality of care delivered to the patient.
- Training new caregivers on caregiver duties and evaluate through return demonstration competency in medication administration and personal care tasks.
- Providing in home supervision of caregivers and provide review of medication regimen at least every 90 days for patient receiving aide services; ensure proper documentation within the client record.
- Reviewing completed MAR each month for completion and errors.
- Creating new monthly MAR based on doctor's current medication orders.
- Delegating nursing services as outlined in policy including assessment of client and caregiver for appropriateness.
- Collaborating with Fedelta team members to facilitate timely office processing of paperwork and personnel information.
- Actively participating in agency quality improvement activities.
- Promoting Fedelta and our services to the public.
- Flexibility with schedule to address urgent needs.
- Providing backup coverage for current RN regarding vacation, sick time off, etc.

Pay: $52 - $55/hour based on experience

• Current unencumbered Oregon RN License
