Home Health Position – Job Details
  • Work Setting: Home visits with periodic office meetings
  • Transportation: Reliable transportation required
  • Driver’s License: Current/valid driver’s license required
  • Insurance: Agency-required liability insurance required
Patient Volume
  • Average Patients/Day: 5–8 patients
  • Types of Cases: Start of Care (SOC), wound care, follow-up visits, and other home health visits
Productivity / Points
  • Points: 30 points per week
  • Each visit/activity has a different point value, including:
    • Patient visits
    • Documentation
    • Travel
    • Other assigned activities
