Home Health Intake Coordinator

Job Summary:

Acute Home Healthcare, a thriving home healthcare agency, is seeking a dedicated and compassionate Home Health Intake Coordinator to join our team. As a key member of our intake team, you will be responsible for processing new referrals, obtaining authorizations, and ensuring timely and efficient coordination of home healthcare services. Responsibilities:

  • Process new referrals in a timely and courteous fashion

  • Enter Authorizations and Schedules in System

  • Obtain Re-authorizations for Additional Homecare Services

  • Follow-up on Requests From the PCP for Services

  • Contact and Receive Authorizations from Insurance Company

  • Contact Clinicians (PT's, OT's, ST's) to Schedule Services

  • Request Home Health Orders From PCP's, Hospitals, and Specialist

  • Review Charts for Procedural Accuracy and Efficiency

  • Receive and Forward Faxes to Correct Departments

  • Confirm Patient's PCP's

  • Assist Patients with Any Questions, Comments, or Concerns

  • Develop Good Working Relationships with PCP Staff and Insurance Companies

  • Availability to work on call on weekends

Experience:

  • Previous experience working in a medical office or healthcare setting preferred

  • Working Knowledge of Microsoft Office (Word, Excel, Outlook, and PowerPoint)

  • Outstanding Customer Service Skills

  • Doctor Office Administrative Experience (Minimum 2-3 years)
