Occupational Therapist (OTR/L) – Home Health

What You'll Do

  • Perform occupational therapy evaluations in patients' homes.
  • Develop individualized treatment plans that improve independence with activities of daily living.
  • Assess home safety and recommend adaptive equipment and environmental modifications.
  • Educate patients and caregivers on techniques that improve safety, function, and quality of life.
  • Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team.
  • Complete timely, accurate documentation while monitoring patient progress.

What We're Looking For

  • Active Minnesota Occupational Therapist (OTR/L) license.
  • Passion for helping patients remain independent at home.
  • Strong communication, organization, and clinical reasoning skills.
  • Home health experience is preferred but not required.
  • Valid driver's license and reliable transportation.
