• Collaborate with primary care providers to recognize, manage, and treat mental health and psychosocial issues, contributing as a vital member of the integrated healthcare team.
• Conduct thorough patient intakes that emphasize diagnostic evaluations and functional assessments, generating tailored treatment recommendations for primary care providers.
• Deliver brief, focused therapeutic interventions for clients requiring immediate mental health support, ensuring efficient care delivery.
• Evaluate clients to determine if they should remain in the primary care setting or be referred to specialized mental health services, maintaining continuity of care throughout the process.
• Implement follow-up strategies to monitor patient progress and adjust treatment approaches as necessary, ensuring optimal outcomes.
• Develop relapse prevention plans for clients, equipping them with strategies to maintain stability and mitigate risks of deterioration.
• Assist in identifying high-risk patients and implement preventive measures to forestall the escalation of their conditions.
• Monitor and coordinate health service delivery related to behavioral healthcare, connecting patients with external treatment providers when necessary.
• Maintain clinical documentation that accurately reflects patient progress and diagnostic information, ensuring compliance with established guidelines.

• A minimum of a Master’s Degree in Clinical Counseling or a closely related field.
