Position Summary Continental Wellness Center is seeking a compassionate and experienced Family Nurse Practitioner (FNP) to provide comprehensive primary care services with a focus on HIV prevention, PrEP management, and care for individuals living with HIV/AIDS . As a Ryan White service provider , our organization delivers integrated medical and behavioral health services to underserved communities. The ideal candidate will have experience working with HIV-positive patients, individuals at high risk for HIV, and patients with co-occurring mental health conditions.

Key Responsibilities

● Provide primary care services to patients, including health assessments, diagnosis, and treatment planning.

● Manage and prescribe HIV Pre-Exposure Prophylaxis (PrEP) and Post-Exposure Prophylaxis (PEP) programs.

● Conduct HIV testing, prevention counseling, and risk-reduction education.

● Provide medical care and ongoing management for patients living with HIV/AIDS .

● Provide medical case management health assessments and service plans

● Monitor lab results, medication adherence, and patient outcomes.

● Coordinate care with mental health providers, case managers, and support services.

● Maintain compliance with Ryan White program requirements and clinical documentation standards.

● Participate in quality improvement initiatives and public health reporting.

● Educate patients on prevention, treatment adherence, and overall wellness.

● Collaborate with community partners to support HIV prevention initiatives.

Qualifications

● Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)

● Active Florida APRN license

● Board certification as Family Nurse Practitioner

● DEA registration

● Experience with HIV care, infectious disease, or PrEP programs preferred

● Experience working with underserved populations

● Knowledge of Ryan White Program standards preferred Skills

● Strong patient-centered communication

● Knowledge of HIV prevention and treatment guidelines

● Cultural competency and trauma-informed care

● Electronic medical record (EMR) proficiency

● Ability to work in an integrated care team

● Bilingual preferred
