The Clinical Pharmacist ensures optimal drug therapy for patients in their clinical specialty area and provides educational activities to the pharmacy staff and other caregivers to insure that patients receive safe and cost effective drug therapy. Reports to: Pharmacy Manager. Responsibilities include but are not limited to:
• Evaluates drug literature by analysis of experimental design and methodology, in order to compare and contrast therapeutic regimens and roles for new drugs, serves as authoritative information resource on drugs and their utilization to promote optimal drug therapy for patients that is appropriate, cost-effective and maximizes clinical outcomes.
• Provides clinical support to the staff pharmacists in their clinical specialty area such as collaborating directly with healthcare providers in resolving medication related issues and providing specific information regarding patients' drug therapy to promote optimal drug therapy
• Designs or develops therapeutic regimens or protocols based on patient specific information in order to maintain safety of drug therapy for patients by utilizing a variety of screening mechanisms.
• Assists in the training of staff such as serving as a preceptor for pharmacy residents and pharmacy students participating in clinical and drug information rotations; serves as a mentor for pharmacy students and residents, contributes as needed at weekly consult rounds, and provides education to nursing staff, physicians and other caregivers through formal and informal presentations in order to support and promote continuous improvement efforts at Hartford Hospital.
• Participates in the Pharmacy clinical support team and in various multidisciplinary patient care committees and participates in drug use evaluations, documents medication interventions, adverse drug reactions and engages in other projects as needed to enhance clinical pharmacy services and promote optimal patient care.
• Participates in system P&T Council and applicable system councils in the development of drug regimens, order sets, and protocols in areas of their expertise.
• Participate in ongoing continuous improvement of department workflow via lean daily management framework.
• Maintains skill and competency to staff in pharmacy (preferably in area of expertise) as needed.
Qualifications:
Education/Licensure/Experience:
  • Current CT Pharmacist license or eligible to receive licensure within 45 days of hire
  • Bachelors in Pharmacy required, PharmD preferred
  • Completion of an ASHP approved pharmacy residency program Or 3 to 5 years pharmacy experience required.
  • BCPS is preferred.
