Registered Nurse, Float

Location/Schedule: The position will float between several of Cascadia's PSRB Residential Treatment Facilities and The Hope Center, all located in Portland, OR. The schedule for this position Monday through Thursday, 8:00 a.m. to 4:30 p.m. Every day brings something new, and your skills are put to meaningful use across a range of care settings.

You'll work Monday through Thursday, 8:00 a.m. to 4:30 p.m.—a consistent weekday schedule with no weekends or nights. Position: Registered Nurse

Program: PSRB Residential & Outpatient

FTE: 0.8 (4 days per week)

Mission: At Cascadia Health, our mission is to deliver whole health care through integrated mental health and addiction services, primary care, and housing. We're dedicated to promoting hope and supporting the well-being of the communities we serve.

Program Description: Cascadia's Psychiatric Security Review Board (PSRB) clinical programs offer diverse trauma-informed treatment services in the community, residential settings, and intensive outpatient settings to individuals mandated to treatment by the State of Oregon's PSRB. Additionally, as a forensic program, we provide monitoring and supervision of the stipulations of conditions of release issued by the PSRB prior to the client returning to the community. The goal of Cascadia's PSRB program is to promote the health, wellness, and safety of the individual within the community.

Staff supervision is available 24 hours per day in PSRB residential group homes. After demonstrating psychiatric stability and community safety, clients may be approved by the PSRB to transition either into a semi-independent living or intensive-case management program where clients live in their own community-based apartments and staff provide daily support with check-ins. Clients in the PSRB program regularly meet with their case managers and attend groups at the day-treatment clinic. Clients can be referred to additional recovery services, such as drug and alcohol services. The program is designed to offer wrap-around supports and clinical services tailored to an individual client's needs. The goal of the continuum of care that Cascadia provides for the PSRB clients, is to assist clients in obtaining wellness and stability, and the highest level possible of independence.

Position Overview: As a Registered Nurse for our PSRB Residential and Outpatient Programs, you'll play a crucial role in coordinating treatment and implementing medical interventions. You'll provide compassionate care and support, ensuring the well-being and safety of our residents. With a focus on trauma-informed care, you'll understand and address the effects of trauma on health and coping. In this role, you'll have flexibility to schedule site visits based on priority and client needs.

Key Responsibilities:

  • Provide high-quality, person-centered nursing care, including medical interventions, monitoring, and timely documentation.
  • Formulate and update Nursing Care Plans based on residents' medical and behavioral health needs.
  • Administer medications safely and perform nursing assessments to determine appropriate interventions.
  • Collaborate with the interdisciplinary team—including clinicians, case managers, and peer support specialists—to develop and implement holistic treatment plans.
  • Work closely with psychiatric prescribers (LMPs) to carry out individualized care plans and monitor medication efficacy and side effects.
  • Act as a liaison with medical staff, pharmacy, and laboratory services to ensure continuity of care.
  • Support transitions of care, including hospital discharges, referrals, and care coordination with outpatient services.
  • Foster community reintegration, recovery, and wellness through strength-based, trauma-informed rehabilitation programming.
  • Maintain accurate and timely documentation in the electronic health record (EHR), including medication reconciliation, progress notes, and incident reports in compliance with regulatory and organizational standards.
  • Participate in regular nursing meetings, case consultations, and ongoing staff development and training.
  • Respond to and support clients during behavioral health crises using de-escalation techniques and trauma-informed care practices.
  • Provide leadership, mentorship, and clinical guidance to residential staff to support safe and effective program operations.
  • Maintain awareness of infection control practices, safety protocols, and participate in quality improvement initiatives.

Qualifications:

  • Active Oregon Registered Nurse License and AHA-Approved BLS/First Aid Certification.
  • Opportunity to become QMHP or QMHA Certified.
  • Minimum two years of clinical experience in inpatient or outpatient settings preferred.
  • Vehicle with unencumbered Driver's License. Ability to travel between Cascadia programs and community locations as needed, eligible for mileage reimbursement.
  • Knowledge of medical conditions commonly encountered by individuals with mental illness.
  • Motivational Interviewing skills and ability to maintain a trauma-informed care environment.
  • Proficiency in computer hardware and software for job functions.
