This position will primarily be remote, which means you will be equipped with the latest technology to engage with patients effectively from your home office. Our community health clinic adheres to an interdisciplinary approach, promoting collaboration among various healthcare providers—including primary care physicians, nurses, and social workers—to create holistic treatment plans that foster overall well-being. You will play a crucial role in bridging mental health care within the primary care framework.

As a Licensed Professional Clinical Counselor, you will:

• Conduct thorough patient intakes to assess immediate mental health needs, followed by diagnostic evaluations and crafting tailored treatment plans in collaboration with primary care providers.
• Deliver focused interventions for patients experiencing mental health challenges, actively supporting their journey towards emotional and psychological stability.
• Assist primary care providers in recognizing, addressing, and managing psychosocial issues, ensuring that every patient is well-informed of their treatment pathway.
• Engage in continuous follow-ups to monitor patients' progress, determining the effectiveness of treatment strategies and making adjustments as necessary.
• Collaborate in a team-oriented environment, sharing insights and recommendations regarding behavioral health services to enhance overall patient care.
• Maintain meticulous clinical records that document patient interactions, treatment progress, and outcomes to guide future care.
• Work closely with other healthcare professionals to develop and execute relapse prevention plans, helping patients achieve stable functioning.

You will work with a diverse population ranging from children to seniors, addressing a variety of mental health concerns including anxiety, depression, trauma, and substance use disorders. The clinic's commitment to confidentiality ensures that each patient feels comfortable sharing their experiences and challenges. To be successful in this role, candidates must possess:

• A minimum of a Master’s Degree in Clinical Counseling or a related field.
• A current Licensed Professional Clinical Counselor (LPCC) license in New York State.
• Strong knowledge of mental health assessment techniques, treatment planning, and therapeutic modalities including family therapy and cognitive-behavioral strategies.
• Exceptional communication and interpersonal skills that foster effective relationships with patients and team members.
• A deep understanding of the socio-economic factors that influence mental health within our community.

While not mandatory, candidates with the following experiences will stand out:

• Proficiency in both English and Spanish to enhance communication with our diverse clientele.
• Previous experience in a community health setting or familiarity with Federally Qualified Health Centers (FQHC).
• Proven ability to collaborate with interdisciplinary teams to optimize patient care.

Focus Counseling Support Services, LLC offers a competitive salary range of USD 87,000 to 105,000 per year, commensurate with experience and qualifications.
