Registered Nurse - Case Manager

Ability to assess, evaluate and drive discharge plan. Coordinate services to the patient's appropriate level of care. Ensure that treatment plan is initiated, evaluated and re-evaluated daily. Follow the progress and lack thereof daily. Monitor length of stay and identify barriers to discharge. Follow Medicare regulatory guidelines regarding patient choice, IMM notices and MOONS.

Schedule: Monday-Friday. Weekend: None. Call: None. Floating: Within facility/scope of practice as needed.

Minimum experience: 3 years - Unit does not accept first time travelers. Clinical screen: Candidate will be reviewed by clinical to determine clinical fit, if clinical pass and within Piedmont guidelines...
