Job description

Registered Nurse Assessment Coordinator – (RNAC/MDS) Float RN

Location: Lancaster and Harrisburg, PA Areas

We are seeking an experienced Registered Nurse Assessment Coordinator – (RNAC/MDS) Float RN to support skilled nursing facilities throughout the Lancaster and Harrisburg areas.

This role provides MDS coverage, support, and training to facilities as needed. The Float MDS Coordinator will work closely with facility teams to ensure accurate assessments, timely documentation, and compliance with applicable state and federal requirements.

Schedule: Full-Time, Flexible Schedule
Travel Requirements: Travel between client facilities may be required. Mileage reimbursement is available.

Primary Responsibilities

MDS Support and Training

  • Assist nursing facilities and MDS Coordinators in completing resident assessments according to state and facility policies

  • Conduct clinical record reviews to ensure accurate and consistent documentation

  • Train and mentor MDS Coordinators on assessment procedures and documentation standards

  • Provide hands-on MDS support to address specific facility needs

  • Assist with MDS coverage when facilities are experiencing staffing needs or vacancies

Auditing and Compliance

  • Identify areas of concern and collaborate with facility teams to develop effective action plans

  • Conduct MDS audits, interventions, and follow-ups based on facility requirements

  • Support accurate and timely completion and submission of MDS assessments

  • Help ensure compliance with MDS, RAI, Medicare, Medicaid, and regulatory requirements

Qualifications
  • Current Registered Nurse (RN) license in Pennsylvania

  • Minimum of one (1) year of experience as an MDS Coordinator/RNAC

  • Strong knowledge of MDS assessments, RAI guidelines, and Pennsylvania Case-Mix

  • Experience in a skilled nursing or long-term care setting

  • Flexibility to travel between multiple client facilities

  • Strong organizational, communication, and clinical assessment skills

Remote and On-Site Flexibility
  • Semi-remote work may be available for candidates with more than five years of MDS experience and Pennsylvania Case-Mix experience
