In this setting, you will serve clients in a hospice facility in Aurora, Illinois, collaborating closely with a multidisciplinary team of healthcare professionals, including nurses, physicians, and case managers. Your role will be pivotal in ensuring a cohesive approach to patient care, promoting seamless communication and collaboration among team members.

• Conduct thorough mental health assessments, including psychosocial evaluations, in-depth diagnostic interviews, and comprehensive risk assessments to understand patients' unique circumstances.
• Develop personalized treatment plans in collaboration with patients and their families, incorporating their specific needs, goals, and cultural considerations to foster effective engagement.
• Provide evidence-based therapeutic interventions such as CBT, DBT, and SFBT in individual, couple, family, and group settings, adapting approaches based on patient responses and feedback.
• Offer immediate crisis intervention services when needed, working alongside other clinicians to ensure effective management of acute distress and maintain continuity of care.
• Ensure accurate and timely clinical documentation, including progress notes, treatment plans, and discharge summaries, adhering to organizational policies and regulatory standards.
• Empower patients and their families through psychoeducation, equipping them with strategies and tools to effectively manage mental health conditions.
• Regularly collaborate with a multidisciplinary team, including psychiatrists, psychologists, and case managers, to coordinate comprehensive and holistic care.
• Conduct ongoing outcome assessments to evaluate treatment effectiveness, utilizing feedback to refine and adjust care plans as necessary.
• Stay informed about current research, best practices, and emergent trends in the mental health field, incorporating new knowledge into clinical practice to enhance patient care.
• Maintain strict confidentiality and adhere to ethical guidelines at all times, ensuring that patient trust and safety remain paramount.

Your primary focus will be supporting hospice patients and their families as they navigate the challenges associated with terminal illnesses. This involves recognizing the emotional and psychological struggles they face, providing not only therapeutic interventions but also compassionate support in making end-of-life decisions.

• Education: A Master’s degree in Social Work is required.
• Licensure: Must possess an active State license as a Licensed Clinical Social Worker (LCSW).
• Skills: Proven ability to work both independently and collaboratively within a multidisciplinary team.
• Communication: Exceptional interpersonal and communication skills, with a talent for building rapport with diverse populations.

• Experience working in hospice or palliative care settings is highly desirable.

• Annual base salary ranging from $70,000 to $100,000, with final compensation determined by geographic location, work experience, and skill level.
