Key Responsibilities

• Act as a liaison between families, outpatient providers, and hospice staff to coordinate comprehensive care plans.
• Engage with insurance companies for pre-certification and utilization management, ensuring seamless access to services for clients.
• Participate actively in discharge and aftercare planning, offering guidance and support tailored to each client's needs.
• Provide therapeutic meal support by modeling healthy relationships with food, addressing emotional and physical needs symbiotically.
• Deliver empathetic and affirming care, ensuring safety for each client while respecting their unique backgrounds and intersecting identities.

Patient Population / Client Focus

In this role, you will primarily work with older adults who may be facing significant life transitions, terminal illnesses, or complex psychosocial challenges. Your contributions will be pivotal in creating a supportive atmosphere that tailors emotional and psychological interventions to enhance client dignity and quality of life during their hospice journey.

Qualifications

• A Master's degree in Social Work (MSW) from an accredited institution.
• A valid Licensed Clinical Social Worker (LCSW) license in the state of Nevada.
• Proven experience in working with older adult populations, particularly in a mental health or hospice care setting.
• Strong clinical and diagnostic skills to assess client needs effectively.
• Exceptional communication and interpersonal skills for strong client relationships.
• A capacity to work independently while also being an effective team player.

Preferred Experience

• Experience in hospice or palliative care settings, with specialized knowledge in geriatric social work.
• Familiarity with electronic health record (EHR) systems and telehealth practices.
• Demonstrated ability to contribute to interdisciplinary teams, enhancing holistic care delivery.

Compensation and Benefits
