Key Responsibilities

• Conduct comprehensive assessments of patients’ mental health conditions, including their emotional, behavioral, and social functioning to establish effective treatment strategies.
• Provide evidence-based psychotherapy to address a range of mental health issues, including but not limited to anxiety, depression, and post-traumatic stress disorder (PTSD).
• Develop and implement individualized treatment plans that take into account the unique cultural and emotional needs of each patient.
• Monitor patient progress over time, adjusting treatment plans as needed based on feedback, assessment results, and evolving patient needs.
• Facilitate communication and collaboration with an interdisciplinary team, including psychiatrists, nurses, and primary care providers, to deliver cohesive care.
• Offer education and support to patients and their families regarding mental health conditions, available treatment options, and community resources.
• Maintain compliance with ethical standards, ensuring patient confidentiality and informed consent are protected at all times.

Patient Population / Client Focus

As an LCSW at Relieve Mental Healthcare, your primary focus will be on individuals who are facing significant emotional and mental health challenges within a hospice care setting. This population often requires a delicate balance of empathy, clinical skill, and understanding to navigate their unique circumstances. You will help patients and their families find peace and understanding during a pivotal time in their lives.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of 3 years of direct experience providing psychotherapy and mental health services in clinical settings.
• Demonstrated clinical skills in conducting assessments and developing treatment plans tailored to individual patient needs.
• Exceptional communication and interpersonal skills to establish rapport with patients and collaborate effectively with a multidisciplinary team.
• Familiarity with electronic health record (EHR) systems and the ability to manage patient documentation efficiently.

Preferred Experience

• Experience working in hospice or palliative care settings is highly desirable.
• Keen understanding of ethical issues and legal obligations in social work practice.
• Familiarity with community mental health resources and support networks.
• Proficiency in innovative therapeutic modalities, such as cognitive-behavioral therapy (CBT), mindfulness techniques or trauma-informed care.

Compensation and Benefits
