Western Reflections Counseling is actively seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) for a full-time remote position. Work Environment

This position is entirely remote, allowing our social workers the flexibility to deliver care from various locations. Despite the virtual setting, you will be an integral part of a team-based approach, collaborating with healthcare professionals, including physicians, nurses, and chaplains, within a hospice center that focuses on holistic care and emotional well-being.

Key Responsibilities
  • Conduct thorough psychosocial assessments to identify patientsโ€™ mental health needs, strengths, and resources, ensuring a comprehensive understanding of their emotional states.
  • Deliver individual, family, and group therapy sessions, employing evidence-based practices tailored specifically to the unique circumstances of patients receiving end-of-life care.
  • Utilize therapeutic modalities such as cognitive-behavioral therapy and solution-focused therapy to help clients and families cope with grief, anxiety, and depression.
  • Develop individualized treatment plans with measurable goals and interventions that are continuously adapted based on ongoing assessments and patient progress.
  • Engage with an interdisciplinary team to ensure integrated care delivery, sharing insights that enhance the quality of mental health support provided.
  • Offer crisis intervention services, including risk assessments and safety plans, to clients and families in distress.
  • Maintain up-to-date and accurate documentation of therapy sessions and client interactions in accordance with ethical and legal standards.
  • Facilitate group therapy sessions, psychoeducation classes, and support groups to foster community and shared healing among clients.
Patient Population / Client Focus

The ideal candidate will work primarily with terminally ill patients and their families, providing essential emotional and psychological support during challenging times. Your role will involve addressing diverse needs, from managing grief and loss to enhancing coping skills and emotional resilience.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • An active state license as a Licensed Clinical Social Worker (LCSW).
  • At least two years of experience in a mental health or outpatient care setting, with a preference for experience in hospice or end-of-life care environments.
  • Comprehensive knowledge of evidence-based therapeutic modalities and an understanding of best practices in social work.
  • Strong skills in interpersonal communication, assessment, and empathy.
Preferred Experience
