) Flagstar Rehab is Seeking Occupational Therapists (OT) for Skilled Nursing Facilities!
Now Staffing Occupational Therapists for Rehab Department in state-of-the-art Skilled Nursing Facilities throughout NYC and Surrounding Areas.
Flagstar Rehab OTs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives.
Flagstar Rehab offers the opportunity for Direct Employment with our agency OR to Be Your Own Boss as an Independent Contractor.

Hourly Pay Rate: W2 $44-$48 1099: $55-$61
Occupational Therapist Job Duties:
  • Provides Occupational Therapy services to residents by assisting in the assessment and treatment, development and implementation of programs,
  • and related professional in-services.
  • Conducts individual, supervised and group treatment as assigned by the Occupational Therapist and as outlined in the treatment plan.
  • Monitors resident's response to intervention and consults with Occupational Therapist if modification is indicated.
  • Adheres to department and resident schedule and modifies as appropriate for treatment regimen.
  • Maintains acceptable resident care rations as determined by the Occupational Therapist.

  • Current NYS OT License and Registration
  • EMR Competence
  • Meets all State and Federal requirements for US employment in a Nursing Facility
  • Malpractice/Liability Insurance (if applicable)
