Your Role

  • Deliver high-quality care to seniors in their homes, group homes, and independent living settings
  • Diagnose and manage complex chronic conditions
  • Collaborate with a multidisciplinary team (physicians, pharmacists, nurses, MAs, and more)
  • Educate patients and families, including discussions on goals of care and end-of-life planning
  • Use critical thinking and evidence-based practices
  • Document thoroughly using modern EMR systems and technology tools
  • Represent At Home Harmony with professionalism and empathy

Qualifications

  • Minimum 2 years as an NP/PA in geriatrics, internal medicine, family practice, or palliative care, preferred
  • Tech-savvy: Familiarity with EMRs (Athena), Microsoft Office, mobile tools, and AI features
  • Experience in home-based care preferred, but not required
  • Graduate from an accredited NP or PA program
  • Active, unrestricted license and national certification
  • BLS, DEA, and prescriptive authority
  • Valid driver's license and reliable transportation
  • Ability to lift up to 50 lbs and navigate stairs
