Physical Therapist - Home Health

Sacramento, California Role: Physical Therapist - Home Health

Responsibilities:

  • Required to prioritize start of care visits
  • Required to supervise physical therapy assistants
  • Next day mandatory reporting of start of care visits
  • Clinical notes and other documentation due within 48 hours of visit
  • Maintain timely communication with supervisor, intake, and office
  • Be generally responsive during normal business hours

Requirement: CA PT License and one year of home health experience

Territory: Greater Sacramento, CA (will be required to expand as needed)

Schedule: 40 hours per week

Training: 3 full days, one in house and two in the field

Documentation System: Kinnser

Equipment Issued: iPad, Verizon One Talk

Mileage Reimbursement: $0.655/mile (from first to last patient)

Parking and Toll Reimbursement: Paid

Company Car: Option with a decreased bill rate

Bill Rate: $105

OT Rate: +$5, paid after 30 points per week

Holiday Rate: +$10

Weekend Rate: N/A

On-Call Rate: N/A

Call Back Rate: N/A

Productivity Requirement: 30 points per week

Point to Hourly Conversion: 1 = 1.33hrs

Caseloads and visit mixes per day will equal to an average of 8 hrs per day and 40 hrs per week. Overtime must be pre-approved by the clinical supervisor.
