Job description

The CHSLD Au cœur de la vie in St-Jérôme is a residence that offers care and services for 24 residents with a physical disability, cognitive loss or significant loss of autonomy. As an indication, under the holders in management, we find a team comprising Nurses, nurses assistant and Beneficiary Attendants as well as an Administrative Assistant in support of management. The CHSLD Au cœur de la vie listens to its residents and employees with the common goal of preserving the values and mission of the company which is to: Carry out its activities in such a way as to respect the service philosophy of the residence for the benefit of the resident, namely: Preserve their dignity; Ensuring their well-being; Support and accompany his family. Main tasks: • Administer the required care to residents; • Provide services adapted to residents' needs; • Maintain regular contact with the doctor as well as the professional services of the CHSLD, if necessary; • Proceed to welcome any new resident in coordination with the Head of the CHSLD department; • Make the link with the resident's family on the state of health of the latter; • Organize medical appointments between the doctor, the professional services and the residents and/or the family, the optionally; • Write the TNP and the care plan: ensure that the care team respects the application of care; • Ensure that the care team completes the FADM and the observation notes for each resident; • Carry out the residents' IIP and ensure their evaluation; • Organize internal meetings between the professionals, the doctor and the care team; • Make information capsules for the care team in coordination with the Head of the CHSLD department; • Make available for management team meetings; • Ensure a continuous link with his fellow members of the management committee; • Keep management informed of the progress of each resident's care; • Preserve the inventory and respect the occupational health and safety policy; • Reinforce the application of the rules, policies, procedures and code of ethics of the Residence. Requirements • Hold a diploma of professional studies in nursing (OIIQ); • CPR, PDSB; • Have a permit to practice from the OIIQ and ensure that it is renewed; • Having accumulated a minimum of 5 years of experience in the field or the equivalent; • Available every other weekend, if necessary.
