Job description

Title: Field Case Management Coordinator-{170377} Location: Illinois- must live in/near the target counties listed: · McLean, De Witt, Macon, Champaign, Douglas, Shelby (and surrounding areas) Pay: $27.70/hr. Schedule: Mon-Fri, 8:00am-5:00pm CST (40 hrs./week) Work Setting: Field-based (travel to member homes) + work-from-home between visits Mileage: Reimbursed Overview We're hiring a Field Case Management Coordinator to support elderly individuals and people with disabilities who are approved for in-home or nursing facility care . This is a field role requiring local travel to members' homes, with documentation and follow-up completed from home between visits.
