What is a Field Care Manager?

A Field Care Manager is a healthcare professional, often a nurse or social worker, who coordinates and manages care for patients outside of hospital settings. They assess patients' needs, develop care plans, and ensure that individuals receive appropriate medical, social, and community services. Field Care Managers often visit clients in their homes or community settings to monitor progress, provide support, and adjust care plans as needed. Their goal is to improve patient outcomes, enhance quality of life, and reduce unnecessary hospitalizations.

What degree do you need for a care manager?

A care manager typically needs at least a high school diploma or equivalent, but many employers prefer candidates with a bachelor's degree in healthcare, social work, nursing, or a related field. Relevant certifications and experience in healthcare or social services can also enhance job prospects and effectiveness in the role.

To thrive as a Field Care Manager, you need a background in nursing, social work, or a related healthcare field, often supported by a relevant degree and licensure. Familiarity with care management software, electronic health records (EHRs), and case documentation systems is typically required. Strong interpersonal communication, organizational skills, and empathy help build trust with patients and coordinate care across providers. These abilities are vital to ensure effective care planning, compliance, and improved health outcomes for clients in community or home settings.
