A Field Care Manager is a healthcare professional, often a nurse or social worker, who coordinates and manages care for patients outside of hospital settings. They assess patients' needs, develop care plans, and ensure that individuals receive appropriate medical, social, and community services. Field Care Managers often visit clients in their homes or community settings to monitor progress, provide support, and adjust care plans as needed. Their goal is to improve patient outcomes, enhance quality of life, and reduce unnecessary hospitalizations.

What degree do you need for a care manager?
