NURSE (BEHAVIORAL HEALTH CARE FACILITATOR)- FT Sill, OK
About the Position: This position is located at Reynolds Army Health Clinic.

Fort Sill is a United States post north of Lawton, Oklahoma, about 85 miles (136.8 km) southwest of Oklahoma City and 50 miless north of Wichita Falls, Texas on Interstate 44. It covers almost 94,000 acres.
The fort was first built during the Indian Wars. It is designated as a National Historical landmark and serves as home of the United States Army Field Artillery School as well as the Marine Corps' site for Field Artillery MOS school, United States Army Air Defense Artillery School, the 31 st Air Defense Artillery Brigade, and the 75 th Field Artillery Brigade. Fort Sill is also one of the four locations for Army Basic Combat Training. It has played a significant role in every major American conflict since 1869.

Responsibilities
Duties:
This position is as a Mental Health Care Facilitator (BHCF). The Primary Care Mental Health Program provides a team-based approach to healthcare by integrating mental health providers and registered nurses into Primary Care Clinics to deliver outstanding, evidence-based biopsychosocial assessment, treatment, prevention and management of mental health problems and biopsychosocially influenced health conditions for Military Health System beneficiaries.
- Care Facilitation and Communication
The Mental Health Care Facilitator performs independent, comprehensive telephone and clinical triaging in the primary care setting, supporting primary care providers in their treatment of patients with mental health conditions. Facilitates continuity of care through a multidisciplinary approach in the treatment of depression, post-traumatic stress disorder (PTSD) and anxiety disorders in adult beneficiaries. Monitors patient progress and symptom severity by conducting a structured telephonic interview and using validated tools (e.g. PHQ-9, GAD-7 and PCL-5). Provides patient education as an integral part of the services offered (e.g. medication education, alcohol effects on mood, etc.). Collaborates with patients to develop SMART self-management goals to assist patients with symptom improvement. Assists patients with treatment plan adherence. Communicates with providers regarding patient status and documents appropriately in the electronic medical record. Recognizes and reports abnormal test results, changes in symptoms and the need for follow-up. Communicates at least weekly with BHC and bi-weekly with the External Mental Health Consultant (EBHC) to review patients that require staffing. Coordinates patient referrals to support services provided through the internal and external community, or the TRICARE network. Responds to emergency situations.
- Education and Caseload Management
Assists clinic with patient and staff education and provides individual and small group training in order to increase referrals and promote understanding of the role of the mental health care facilitator. Engages in activities to increase caseload (e.g. reviewing PCM appointments, reviewing population metrics to target patients that would benefit from care facilitation etc.). Assists Mental Health Consultant (BHC) with educational classes. Compiles and maintains a variety of data reports, submitting detailed and aggregate program and caseload data reports as needed by program staff. Participates in hospital and clinic meetings, and performance improvement activities as assigned. Works collaboratively with the Nurse Case Manager to provide comprehensive, coordinated care.
- Quality Assurance
Follows quality improvement and internal control activities for the ambulatory care setting to include risk appraisals, conducting chart reviews, identifying trends and reporting corrective actions. Engages in at least quarterly peer review of chart documentation with other BHCF(s).
Perform other duties as assigned.

Basic Requirement for Nurse (Behavioral Health Care Facilitator):
Degree: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Note: You must attach a copy of your transcripts.). License: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
Foreign Degree: A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.

Education/Experience: A diploma, associate degree, or bachelor's degree in nursing from a professional nursing program and possession of at least one full year of professional nursing experience in an ambulatory or inpatient setting (equivalent to the GS-9 grade level or above) which includes supporting patient adherence to treatment plans; providing patient education; and assessing patient symptoms against standard measures.

Education: A PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. (Note: You must attach a copy of your transcripts.).
