Fertility Patient Care Navigator

The Fertility Patient Care Navigator plays an integral role in supporting patients on their journey to parenthood. If you are compassionate, detail-oriented, and thrive in a fast-paced environment, this role is an excellent opportunity to make a meaningful difference. Working closely with a multidisciplinary team, you'll guide patients through their care plans, ensuring they receive the support and resources they need every step of the way.

We are seeking a Fertility Patient Care Navigator to join our team at Pinnacle Fertility - Ohio in Westerville, OH. This is a full-time, onsite position, working Monday through Thursday from 8:00 AM to 4:00 PM, and Fridays from 8:00 AM to 2:00 PM. One weekend per month and participation in a holiday rotation is required.

Key Responsibilities

  • Serve as the primary contact for patients, guiding them through treatment plans and educating them on procedures, testing, and follow-up care.
  • Coordinate and schedule patient appointments, tests, and referrals to ensure seamless care.
  • Assist patients in understanding their medical care, insurance coverage, and available financial resources.
  • Collaborate with healthcare providers, clinical staff, and administrative teams to ensure continuity of care.
  • Maintain accurate and up-to-date patient records in the electronic medical record (EMR) system.
  • Provide emotional support to patients and families, addressing concerns and barriers to care.
  • Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
  • Educate patients on treatment protocols, self-care strategies, and medication usage.
  • Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
  • Advocate for patients' needs and assist in resolving barriers to care.
  • Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.
  • Perform other duties and projects as assigned.

Position Requirements

  • Education & Experience: High school degree required. Minimum of 2 years in patient navigation, case management, or a related healthcare role. Certified Medical Assistant is a plus.
  • Skills: Strong knowledge of healthcare systems, insurance processes, and patient advocacy. Excellent communication, organizational, and problem-solving skills. Proficiency with electronic medical records (EMR) and healthcare software. Empathy and a patient-focused approach, with the ability to work collaboratively in a multidisciplinary team. Bilingual skills are a plus.

Compensation & Benefits

  • Hourly Rate: $22.00 - $26.00 per hour (final compensation based on experience, skills, and qualifications).
