GENERAL SUMMARY:
Under the direction of the Clinical Director, Behavioral Health Services, the social worker provides psychosocial services to patients and families of all ages with particular emphasis on the emotional and social issues resulting from illness, disability, and/or death.

These duties are to be performed in a highly confidential manner, in accordance with the mission, values and behaviors of Mercy Hospital and Medical Center. Employees are further expected to provide a high quality of care, service, and kindness toward all patients, staff, physicians, volunteers and guests.


Essential Duties amp; Responsibilities:
  • Documents psychosocial history, initial assessment, appropriate interventions, and expected discharge plan in a comprehensive, clear, timely and legible manner.
  • Reassessment automatically occurs upon notification of significant change in patient’s health status.
  • Coordinates with government agencies.
  • Assists in the development, planning, and facilitation of therapeutic and educational groups for patients/families.
  • Provides clinical social work assessments and services (information, referrals, and counseling) to individuals and/or families within the Mercy Hospital and Medical Center system as well as the general community.
  • Provides services to the patients/families in a professional manner in keeping with the philosophy and mission and principles stated in the Code of Ethics of the National Association of Social Workers.
  • Maintains knowledge of community resources that will enable patients and families to use the health care system and other organizations to their greatest benefit.
  • Provides consultation to the health care team regarding psychosocial management of patient problems or job-related stresses.

Qualifications:
  • Master Degree in Social Work from an accredited school required.
  • Licensed as a Social Worker in the State of Illinois required.

  • Ability to cope with stress.
